Amount $2,500
No monies will be given directly to student. Scholarships will be sent directly to the college being attended.
Requirements:
- Has received an Organ Transplant
- Colorado Resident (no age requirements; can go to out of state college)
- Attending College Full-time in the Fall of 2024
- 2.5 GPA or higher
Information needed:
- Name
- Address
- Phone number
- School Presently Attending
- Recent Transcript showing GPA
- Name and Address of College attending/to attend (with name/contact info of admissions officer, copy of registration with student ID#)
- Please list (with year of involvement): school activities, awards, honors, employment, community service/volunteering, or other activities
- Submit a 300 word essay on your transplant story
